Duplicate passports in lieu of lost / stolen
passports
In case of loss of passport in Peru
or Bolivia, a duplicate passport is normally issued
only if the person is living abroad or plans to visit
other countries before returning to India. Otherwise,
an Emergency Certificate is issued, which permits the holder to return to India
directly.
In case the lost / stolen passport
has not been issued by this Embassy, the duplicate passport
issued will only be of short validity (1 year), which
has to be renewed to the original validity of the lost
passport subsequently after receiving clearance from
the original Passport Issuing Authority. This can take
up to 3 months. Alternatively, for persons on short
visits, the duplicate passport can be renewed directly
from the original Passport Issuing Authority.
The following documents are required:

Application
form for New Passport [168 kb. *.doc] complete
in all respects. The application form has to be
signed at four different places, including the declaration.
Without this, no service can be rendered.

Proof of Indian citizenship,
preferably a photocopy of the lost passport if available.
If not, at least the old passport Number, place
and date of issue are required.

In case person is resident in
Peru / Bolivia, copy of Carnet de Extranjería.
If not resident in Peru / Bolivian, Airlines Tickets
and Itinerary. In case none of these are available,
the applicant must furnish a detailed explanation
by way of sworn affidavits of his current Visa status,
how he travelled abroad and entered Peru / Bolivia,
and details of travel document used.

FIR or local Police Report recording
the loss of passport.
- Four (4) photographs, of which
one is to be pasted on page 1 of the form, and signed

The corresponding fees (US$
153 for 36
pg booklet and US$ 178 for 60 pg booklet)
** Only for indians residing in Peru
If none of the two of 2 above available, then applicant
must produce some other document of Identity such as:
a) Driving Licence.
b) Carnet de Extranjería / Identity card.
c) Letter from Employer certifying Identity of the
applicant.
d) Sworn Affidavits from two Indian Nationals resident
in Peru / Bolivia in respect of the Identity and citizenship
of the applicant may also be obtained.
