Damaged passports
Issue of fresh passports in lieu of
damaged passports will be treated as a case of lost
passport and similar fees will be levied.
The following documents are required:

Application form for New Passport [168 kb. *.doc]
complete in all respects. The application form has
to be signed at four different places, including
the declaration. Without this, no service can be
rendered.

Damaged Passport in original.

Six (6) photographs, of which
one is to be pasted on page 1 of the form, and signed
half on the form and half on the photograph.
