Damaged passports

Issue of fresh passports in lieu of damaged passports will be treated as a case of lost passport and similar fees will be levied.

The following documents are required:

• Application form for New Passport [168 kb. *.doc] complete in all respects. The application form has to be signed at four different places, including the declaration. Without this, no service can be rendered.
• Damaged Passport in original.
• Six (6) photographs, of which one is to be pasted on page 1 of the form, and signed half on the form and half on the photograph. clic here REQUIREMENTS OF PHOTOGRAPH

• The corresponding fees (US$ 128 for 36 pg booklet and US$ 143 for 60 pg booklet)
