Change of name

A. Once a passport has been issued in a particular name, any change of name, other than by marriage / divorce / remarriage, the following documents are required. A new booklet will be issued, either of 36 pgs or 60 pgs.


Miscellaneous application form [115 kb. *.doc], to be filled and signed in four places including the declaration, without which no service will be rendered.

Passport in original.

Four photographs, one to be pasted on page 1 of the form and signed half on the form and half on the photograph.

Sworn affidavit in presence of the consular officer at the Embassy (or notarised along with the address and identification number of the notary) as per this format.

Original newspaper cuttings announcing the change of name published in one leading Peruvian / Bolivian daily and the other in a leading daily in India near the applicant's Indian address.

The corresponding fees (US$ 20 for 36 pg booklet and US$ 25 for 60 pg booklet) plus US$ 10 in case of affidavit at the Embassy. However, in case this service is being requested at the time of issue of new passport or booklet, only the fees for US$ 10 (in case of affidavit at the Embassy) is applicable.

B. For change of name after marriage, the following documents are required. A new booklet will be issued, either of 36 pgs or 60 pgs. This procedure can also be used for insertion of spouse's name in the passport after marriage.

Passports, in original, of both husband and wife.

Four photographs, one to be pasted on page 1 of the form and signed half on the form and half on the photograph.

Original marriage certificate (issued by the Registrar of Marriages) or an attested copy of the same. Marriage certificate issued by a Church or Quazi (with a certified translation) is also acceptable.

The corresponding fees (US$ 20 for 36 pg booklet and US$ 25 for 60 pg booklet) However, in case this service is being requested at the time of issue of new passport or booklet, no fees are required.

 
Note: In case a marriage certificate is not available, a joint affidavit in the presence of the consular officer (or notarised along with the address and identification number of the notary) is required, along with a joint photograph. The format for the affidavit is available at the Embassy. In case of affidavit at the Embassy, an extra fee of US$ 10 would be applicable.
 

C. For change of name after divorce (or death of spouse) / remarriage, a similar procedure as for change of name after marriage would be followed. However, a divorce certificate, authenticated by the court (or copy of death certificate), is required. A new booklet will be issued, either of 36 pgs or 60 pgs. Detailed procedures regarding this service can be obtained directly from the Embassy.
